摘要/Abstract

摘要： 目的: 对比单切口尿道中段无张力悬吊术（AJUST）和三切口经阴道闭孔路径尿道中段无张力悬吊术（TVT-O）治疗女性压力性尿失禁（SUI）的临床效果。方法: 52例女性SUI患者分别采用AJUST和TVT-O两种术式治疗,比较两组患者的围手术期参数,以及术后的主观疗效、客观疗效,采用ⅡQ-7简表评价患者术后生活质量改善情况。结果: 两组患者留置尿管时间、住院时间、术前和术后ⅡQ-7评分,以及术后3、6、12个月的主观治愈率、客观治愈率比较,差异均无统计学意义（P>0.05）。AJUST组手术时间、术中出血量、穿刺路径疼痛发生率均明显低于TVT-O组（P<0.05）;AJUST组患者术后7d主观治愈率明显高于TVT-O组（P<0.05）。结论: TVT-O和AJUST治疗女性中重度SUI的疗效相当,但AJUST手术操作简单、时间短、安全性高,值得临床推广应用。

Abstract: Objective: To compare the clinical effects of AJUST and TVT-O for treating female stress urinary incontinence (SUI). Methods: 52 female SUI patients were respectively treated with AJUST and TVT-O. The perioperative parameters, subjective curative effects, objective curative effects, improvement of life quality (ⅡQ-7 score) of patients in 2 groups were compared. Results: There was no significant difference of catheterization time, hospital days, ⅡQ-7 score before and after operation, as well as subjective cure rate and objective cure rate 3 months, 6months, 12 months after operation between 2 groups (P>0.05). The operation time, intraoperative blood loss and incidence of puncture pain of patients in AJUST group were obviously lower than TVT-O group (P<0.05); The subjective cure rate of patients in AJUST group 7d after operation was obviously higher than TVT-O group (P<0.05). Conclusions: The curative effect of TVT-O and AJUST in the treatment of moderate to severe female SUI is equivalent; Moreover, AJUST has the advantages of simple operation, short operation time and high security, so it is worthy of application in clinic.
