基于BIS的闭环TCI靶控输注丙泊酚在老年骨折患者手术中的应用

APPLICATION OF BIS-BASED CLOSED-LOOP TCI TARGET-CONTROLLED INFUSION OF PROPOFOL IN SURGERY OF ELDERLY FRACTURE PATIENTS

摘要: 目的:探讨基于脑电双频指数(BIS)的闭环靶控输注(TCI)丙泊酚用于老年骨折手术的效果。方法:80例择期行骨折手术的老年患者随机分为A组(40例)和B组(40例),A组采用BIS指导下的闭环TCI输注丙泊酚,B组采用恒速输注丙泊酚。比较两组患者围术期各项指标及麻醉诱导前(T1)、气管插管时(T2)、手术开始切皮时(T3)、手术开始后30分钟(T4)、手术结束(T5)时5个时间点的BIS值、心率(HR)及平均动脉压(MAP),以及围术期事件的发生情况。结果:A组患者丙泊酚用量、苏醒时间及拔除气管导管时间均显著低于B组(P<0.05);T2-T5时刻,A组的BIS值、MAP显著高于B组(P<0.05),HR均显著低于B组(P<0.05);A组的围术期事件发生率(5.00%)显著低于B组(20.00%)(P<0.05)。结论:基于BIS的TCI输注丙泊酚应用于老年骨折手术能降低患者的应激反应水平,减少丙泊酚用量,有效提高麻醉的安全性。

Abstract: Objective: To explore the effects of closed-loop target-controlled infusion (TCI) of propofol based on bispectral index (BIS) in surgery of elderly fracture patients. Methods: 80 elderly fracture patients were randomly divided into group A and group B with 40 patients in each group. The patients in group A were given propofol with BIS-based closed-loop TCI target-controlled infusion, while the patients in group B were given propofol with constant-speed infusion. The perioperative indicators and the incidence of perioperative events, as well as BIS, heart rate (HR) and mean arterial pressure (MAP) at five time point (T1 before anesthesia induction, T2 endotracheal intubation, T3 skin incision, T4 30 minutes after operation, T5 at the end of operation) of patients in 2 groups were compared. Results: The propofol dosage, recovery time and the time of tracheal extubation of patients in group A were obviously lower than group B (P˂0.05). At time point T2 to T5, the BIS and MAP of patients in group A were obviously higher than group B (P˂0.05), the HR were obviously lower than group B (P˂0.05). The incidence rate of perioperative events of group A was obviously lower than group B (5.00% vs 20.00%, P˂0.05). Conclusions: BIS-based closed-loop TCI target-controlled infusion of propofol can reduce the stress level surgery of elderly fracture patients and the propofol dosage, and also can improve the safety of operation effectively.
