肥胖型男性2型糖尿病患者性激素和骨钙素水平的变化及其与IR的相关性

CHANGES OF SEX HORMONES, OSTEOCALCIN LEVEL IN OBESE MALE T2DM PATIENTS AND THEIR CORRELATION WITH INSULIN RESISTANCE

摘要: 目的 探讨肥胖型男性2型糖尿病(T2DM)患者性激素、血清骨钙素(OC)水平的变化情况及其与胰岛素抵抗(IR)的相关性。方法 以我院收治的肥胖型男性T2DM患者100例为T2DM组,另选45例年龄相仿、同期来我院体检的健康男性作为对照组。记录所有受试者的一般临床资料,检测性激素水平,包括促卵泡激素(FSH)、黄体生成素(LH)、雌二醇(E2)、睾酮(T);检测OC、空腹血糖(FBG)、餐后2小时血糖(2hPBG)、空腹胰岛素(FINS)水平,并计算胰岛素抵抗指数(HOMA-IR);采用Pearson相关性分析分析性激素、OC水平与HOMA-IR的相关性。结果 T2DM组患者的FBG、2hPBG、FINS、HbA1c、HOMA-IR、E2水平均显著高于对照组(P<0.05);OC水平、T水平显著低于对照组(P<0.05)。肥胖型男性T2DM患者血清E2水平与HOMA-IR成正相关关系(r=0.172,P<0.05),血清T水平、OC水平与HOMA-IR成负相关关系(r=-0.166、r=-0.216,P<0.05)。结论 肥胖型男性T2DM患者OC水平、E2水平、T水平与IR有关,改善肥胖型男性T2DM患者OC及性激素水平对于改善IR情况、缓解病情可能具有积极意义。

Abstract: Objective: To explore the changes of sex hormones and serum osteocalcin (OC) level in obese male patients with type 2 diabetes mellitus (T2DM) and their correlation with insulin resistance (IR). Methods: 100 obese male patients with T2DM who were admitted in our hospital were enrolled as T2DM group; 45 healthy males with similar age who underwent physical examination in the hospital during the same period were enrolled as control group. The general clinical data of all the subjects were recorded. The level of sex hormone were detected, including follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2) and testosterone (T). The level of OC, fasting blood glucose (FBG), 2-hours postprandial blood glucose (2hPBG) and fasting insulin (FINS) were detected. The homeostasis model assessment of insulin resistance (HOMA-IR) was calculated. Pearson correlation analysis was used to analyze correlations between sex hormone, OC level and HOMA-IR. Results: The FBG, 2hPBG, FINS, HbA1c, HOMA-IR and E2 of patients in T2DM group were significantly higher than control group (P<0.05); While the OC level and T level were significantly lower than control group (P<0.05). The E2 level of obese male T2DM patients was positively correlated with HOMA-IR (r=0.172, P<0.05); While the T level and OC level were negatively correlated with HOMA-IR (r=-0.166, -0.216, P<0.05). Conclusions: The level of OC, E2 and T of obese male T2DM patients are related to IR. Improving OC and sex hormones levels in obese male T2DM patients may be of positive significance for improving IR and relieving disease.
