IL-17、PLR、RF在类风湿关节炎诊断中的应用及临床意义

摘要: 目的 探究白细胞介素17(IL-17)、外周血血小板/淋巴细胞比率(PLR)、类风湿因子(RF)在类风湿关节炎(RA)诊断中的应用及临床意义。方法 选取于2017年6月~2019年12月在我院住院治疗的RA患者78例作为研究组,依据28关节疾病活动指数(DAS28)评分将78例RA患者分为30例活动期组和48例缓解期组,同时选取78例健康者作为对照组。记录所有患者血清IL-17、PLR、RF水平,并分析其在RA诊断中的应用及临床意义。结果 研究组的IL-17、PLR、RF水平均高于对照组(P<0.05);活动期组的IL-17、PLR、RF水平均高于缓解期组(P<0.05);ROC结果显示:IL-17、PLR以及RF单独及联合诊断的AUC分别为0.800、0.878、0.854、0.964,联合诊断的AUC高于单项诊断(P<0.05)。结论 通过检测血清IL-17、PLR、RF水平既可反映病情的进展,联合诊断又可提高RA诊断的准确率,对RA患者的诊治具有重要的临床价值。

Abstract: Objective To explore the application and clinical significance of interleukin 17 (IL-17), peripheral blood platelet/lymphocyte ratio (PLR) and rheumatoid factor (RF) in the diagnosis of rheumatoid arthritis (RA). Methods A total of 78 RA patients who were hospitalized in the hospital from June 2017 to December 2019 were enrolled as study group. According to the disease activity score 28 (DAS28), they were divided into active stage group (30 cases) and remission stage group (48 cases). And 78 healthy people were enrolled as control group. The levels of serum IL-17, PLR and RF in all patients were recorded. And their application and clinical significance in the diagnosis of RA were analyzed. Results The levels of IL-17, PLR and RF in study group were higher than those in control group (P<0.05), which were higher in active stage group than remission stage group (P<0.05). ROC results showed that AUC values of IL-17, PLR, RF and their combination for diagnosis of RA were 0.800, 0.878, 0.854 and 0.964, respectively. AUC of combined diagnosis was higher than that of single index (P<0.05). Conclusion The detection of serum IL-17, PLR and RF levels can reflect disease progress, and their combination improve diagnosis accuracy rate of RA, which is of important clinical value in the diagnosis and treatment of RA patients.
