老年慢性心力衰竭患者血清Nexilin、Beclin-1、CTRP5水平的变化及临床意义

Changes and Clinical Significance of Serum Levels of Nexilin, Beclin-1 and CTRP5 in Elderly Patients with Chronic Heart Failure

摘要: 目的 探究老年慢性心力衰竭(CHF)患者血清微管连接蛋白(Nexilin)、自噬效应蛋白(Beclin-1)、补体Clq/肿瘤坏死因子相关蛋白5(CTRP5)水平的变化及临床意义。方法 选取南阳市康复医院2022年3月~2023年3月收治的老年CHF患者88例为观察组,按照纽约心脏病学会(NYHA)心功能分级将其分为Ⅱ级、Ⅲ级、Ⅳ级,根据治疗6个月后患者预后情况将其分为预后良好者、预后不良者,另选取同期健康体检者88名为对照组,对比2组、不同心功能分级、不同预后患者血清Nexilin、Beclin-1、CTRP5水平,分析血清Nexilin、Beclin-1、CTRP5水平与心功能分级的相关性,各指标联合检测对患者预后不良的预测价值。结果 观察组血清Nexilin、Beclin-1、CTRP5水平明显高于对照组(P<0.05);心功能Ⅳ级患者血清Nexilin、Beclin-1、CTRP5水平明显高于Ⅲ级、Ⅱ级患者,Ⅲ级患者高于Ⅱ级(P<0.05);血清Nexilin、Beclin-1、CTRP5水平与心功能分级呈正相关(P<0.05);与预后良好者相比,预后不良者血清Nexilin、Beclin-1、CTRP5水平更高(P<0.05);血清Nexilin、Beclin-1、CTRP5水平联合预测预后不良曲线下面积(AUC)为0.846,约登指数为0.491,敏感度为72.22%,特异度为76.92%(P<0.05)。结论 血清Nexilin、Beclin-1、CTRP5水平与老年CHF关系密切,可为临床预测老年CHF提供有效参考。

Abstract: Objective To explore the changes and clinical significance of serum levels of microtubule-associated protein (Nexilin), Recombinant human Beclin 1 protein (Beclin-1), Complement Clq/tumor necrosis factor related protein 5 (CTRP5) in elderly patients with chronic heart failure. Methods A total of 88 elderly patients with chronic heart failure admitted to Nanyang Rehabilitation Hospital from March 2022 to March 2023 were selected as the observation group, they were divided into grade II, III, and IV according to the New York Heart Association (NYHA) classification of cardiac function. According to the prognosis of the patients after 6 months of treatment, they were divided into good prognosis and poor prognosis groups. In addition, 88 healthy physical examination subjects in the same period were selected as the control group. The levels of serum Nexilin, Beclin-1, and CTRP5 were compared between the two groups, different cardiac function grades, and different prognosis patients. The correlation between serum Nexilin, Beclin-1, and CTRP5 levels and cardiac function grades was analyzed, and the predictive value of combined detection of various indicators for poor prognosis was analyzed. Results The levels of serum Nexilin, Beclin-1, and CTRP5 in the observation group were significantly higher than those in the control group (P<0.05); the levels of serum Nexilin, Beclin-1, and CTRP5 in patients with cardiac function grade IV were significantly higher than those in patients with cardiac function grade III and II, and the levels in patients with cardiac function grade III were higher than those in patients with cardiac function grade II (P<0.05); the levels of serum Nexilin, Beclin-1, and CTRP5 were positively correlated with the classification of cardiac function (P<0.05). Compared with patients with good prognosis, patients with poor prognosis had higher levels of serum Nexilin, Beclin-1, and CTRP5 (P<0.05). The area under the curve (AUC) of the combined prediction of poor prognosis using serum levels of Nexilin, Beclin-1, and CTRP5 was 0.846, with a Youden index of 0.491, a sensitivity of 72.22%, and a specificity of 76.92% (P<0.05). Conclusion The levels of serum Nexilin, Beclin-1, and CTRP5 are closely related to elderly chronic heart failure, which can provide effective reference for clinical prediction of elderly chronic heart failure.
