探讨双源高分辨螺旋CT颈部血管造影对脑梗死的诊断价值

The Diagnostic Value of Double-source High Resolution Spiral CT Angiography in Cerebral Infarction

摘要: 目的 比较双源高分辨螺旋CT及彩超在脑梗死患者颈动脉病变诊断中的应用价值。方法 选取2018年1月~2019年12月我院收治的102例脑梗死患者,使用双源高分辨螺旋CT和彩超对患者的颈动脉狭窄程度、颈动脉不同部位的血管斑块检出情况及颈动脉斑块类型进行对比分析。结果 双源高分辨螺旋CT和彩超对脑梗死患者的颈动脉血管狭窄程度的分析基本一致,无显著差异。但双源高分辨螺旋CT对脑梗死患者的颈内动脉血管及颈外动脉血管的斑块的检出数量显著高于彩超组,差异有统计学意义(χ2=16.06,P=0.0001;χ2=18.73,P=0)。同时双源高分辨螺旋CT对软斑及溃疡斑的检出数量也显著高于彩超组(χ2=9.16,P=0.0025;χ2=9.48,P=0.0021)。结论 双源高分辨螺旋CT及彩超均对颈动脉血管狭窄程度检测具有很好的灵敏性,但是双源高分辨螺旋CT能够对颈内不同部位的血管斑块及斑块的类型进行深入分析,对脑梗死的诊断具有较高的应用价值。

Abstract: Objective To compare the application value of dual-source high resolution spiral CT and color ultrasound in the diagnosis of carotid artery lesions in patients with cerebral infarction. Methods A total of 102 patients with cerebral infarction admitted to our hospital from January 2018 to December 2019 were selected for comparative analysis with dual-source high-resolution spiral CT and color ultrasound on the degree of carotid artery stenosis, detection of vascular plaques in different parts of the carotid artery and types of carotid artery plaques. Results The analysis of the degree of carotid artery stenosis in patients with cerebral infarction by dual-source high resolution spiral CT and color ultrasound was basically the same without significant difference. However, the number of plaques in internal carotid artery and external carotid artery vessels detected by dual-source high-resolution spiral CT in patients with cerebral infarction was significantly higher than that in the color ultrasound group, showing a statistical difference (χ2=16.06, P=0.0001; χ2=18.73, P=0). At the same time, the number of soft spots and ulcerative spots detected by dual-source high resolution spiral CT was significantly higher than that of the color doppler ultrasound group (χ2=9.16, P=0.0025; χ2=9.48, P=0.0021). Conclusion Dual source high resolution spiral CT and color doppler ultrasound have good sensitivity to the degree of carotid artery stenosis. But dual source high resolution spiral CT can carry out in-depth analysis on the vascular plaques and types of plaques in different parts of the neck, which has a high application value in the diagnosis of cerebral infarction.
