2型糖尿病及其慢性并发症患者甲襞微循环的变化及意义

CHANGES AND SIGNIFICANCE OF NAIL FOLD MICROCIRCULATION IN PATIENTS WITH TYPE 2 DIABETES MELLITUS AND ITS CHRONIC COMPLICATIONS

摘要: 目的: 探讨2型糖尿病及其慢性并发症患者甲襞微循环的变化与意义。方法: 以103例2型糖尿病患者作为观察组,经系统检查后分为视网膜病变组(DR组)和正常眼底组(NDR组)、糖尿病肾病组(DN组)和正常蛋白尿组(NDN组),糖尿病周围神经病变组(DPN组)和无糖尿病周围神经病变组(NDPN组),糖尿病下肢血管病变组(PAD组)和无糖尿病下肢血管病变组(NPAD组);选取同期30例健康体检者作为对照组。测定所有受试者甲襞微循环情况。结果: 2型糖尿病患者甲襞微循环各项积分和总积分均显著高于健康体检者(P<0.01)。2型糖尿病并发DR、DN、DPN、PAD的患者甲襞微循环各项积分和总积分均显著高于无相应并发症的2型糖尿病患者(P<0.01,P<0.05)。结论: 2型糖尿病患者存在甲襞微循环障碍,且并发DR、DN、DPN、PAD的患者甲襞微循环障碍情况更严重,临床上应尽早行甲襞微循环检测,以指导临床更好地防治2型糖尿病及其慢性并发症。

Abstract: Objective: To investigate the changes and significance of nail fold microcirculation in patients with type 2 diabetes mellitus and its chronic complications. Methods: 103 patients with type 2 diabetes mellitus were taken as observation group in this study; and the 103 type 2 diabetes mellitus patients were further divided into diabetic retinopathy (DR) group and no DR (NDR) group, diabetic nephropathy (DN) group and no DN (NDN) group, diabetic peripheral neuropathy (DPN) and no DPN (NDPN) group, peripheral arterial disease (PAD) group and no PAD (NPAD) group. 30 cases of healthy controls at the same term were taken as control group. The nail fold microcirculation of all the subjects were detected. Results: The nail fold microcirculation scores and total scores of type 2 diabetes mellitus patients were all obviously higher than healthy controls (P<0.01). The nail fold microcirculation scores and total scores of DR, DN, DPN, PAD patients were obviously higher than NDR, NDN, NDPN, NPAD patients respectively (P<0.01, P<0.05). Conclusions: There is a disturbance of nail fold microcirculation in type 2 diabetes mellitus patients, especially those complicated with DR, DN, DPN and PAD. In clinic, the nail fold microcirculation should be detected early, in order to better guide clinical prevention and teratment of type 2 diabetes mellitus and its chronic complications.
