Abstract: Objective: To analyze the serum protein markers in diabetic nephropathy (DN) patients with poor prognosis in stage III-IV using SELDI-TOF-MS technique. Methods: 50 DN patients in stage III-IV were divided into poor prognosis group (n=27) and good prognosis group (n=25); And 25 health examinees in the same period were tanken as control (normal control group). The serum of all the subjects were obtained; SELDI-TOF-MS was used to establish the protein fingerprint and analyze the differential protein among different groups. Results: There were two highly expressed differential proteins between the good prognosis group and the normal control group; There were four highly expressed differential proteins between the good prognosis group and the poor prognosis group; There were five highly expressed differential proteins between the poord prognosis group and the normal control group. Conclusions: The protein with a mass-charge ratio (M/Z) of 3125.76Da may be the serum protein marker of DN patients with poor prognosis in stage III-IV.

摘要： 目的:应用表面增强激光解吸/电离飞行时间质谱技术（SELDI技术）分析Ⅲ期-Ⅳ期预后不佳糖尿病肾病(DN)患者的血清蛋白标志物。方法:50例Ⅲ期-Ⅳ期DN患者,分为预后不佳组27例和预后良好组23例,以同期25例健康体检者作为对照。提取所有受试者的血清标本,采用SELDI-TOF-MS技术建立所有受试者的血清蛋白质指纹图谱,通过组间比较筛选出差异蛋白。结果:DN预后良好组与正常对照组比较,存在2个呈现高表达状态的差异蛋白;DN预后良好组与预后不佳组比较,存在4个呈现高表达状态的差异蛋白;DN预后不佳组与正常对照组比较,存在5个呈现高表达状态的差异蛋白。结论:质荷比（M/Z）为3125.76Da的蛋白可能是Ⅲ期-Ⅳ期DN患者预后不佳的血清蛋白标志物。
