Diagnostic Efficacy of Schizas Morphological Classification and Dural Sac Area Measurement in Lumbar Spinal Stenosis

Schizas形态学分型和硬膜囊面积测量对腰椎管狭窄症的诊断效能

Abstract: Objective To evaluate the efficacy of Schizas morphological classification and dural sac area measurement in the diagnosis of lumbar spinal stenosis. Methods Eighty-four patients with low back pain were randomly selected as subjects. Schizas system and dural sac area measurement were used to grade the results of lumbar MRI. The consistency and reliability of the results between the two groups were compared. According to the clinical diagnosis results of the patients, the accuracy, sensitivity and specificity of the two grades were analyzed. Results The consistency between the two methods of Schizas system and dural sac area measurement was excellent. Both classification methods have good retest reliability. The accuracy, sensitivity and negative predictive value of Schizas system were significantly higher than those of DSCA grading system. Conclusion Schizas system and DSCA measurement are reliable grading systems, and both methods can be used to evaluate lumbar spinal stenosis. Compared with DSCA measurement, Schizas system is more efficient in clinical diagnosis.

摘要: 目的 探讨Schizas形态学分型和硬膜囊面积测量对腰椎管狭窄症的诊断效能。方法 随机选择因腰腿痛就诊的84例患者为研究对象,使用Schizas形态学分型和硬膜囊面积测量2种方法对腰椎磁共振结果进行分级,比较两组结果的观察者间一致性和复测可靠性。根据患者的临床诊断结果,分析2种分级的准确率、敏感度、特异性。结果 Schizas形态学分型和硬膜囊面积测量2种方法的组间一致性良好。2种分级方法的复测可靠性良好。Schizas分型诊断结果准确率、敏感度、阴性预测值均高于测量分级诊断(P<0.05),差异有统计学意义。结论 Schizas分型和硬膜囊面积测量都是可靠的分级系统,2种方法均可用于评估腰椎管狭窄症。与硬膜囊面积测量相比,Schizas分型的临床诊断效能更高。
