Diagnostic Value of HGB, SF and TfR in Gestational Thalassemia

HGB、SF、TfR在妊娠期地中海贫血中的诊断价值

Abstract: Objective To explore the diagnostic value of serum hemoglobin (HGB), ferritin (SF) and transferrin receptor (TfR) in patients with mild thalassemia in the second and third trimesters of pregnancy. Methods During the period from August 2017 to July 2019, 40 patients with mild thalassemia in the second trimester of pregnancy and 40 patients in the third trimester of pregnancy who received treatment in our hospital were enrolled, another 40 healthy people during the same period were enrolled as healthy control group. The levels of serum HGB, SF and TfR were compared among the three groups. The correlation among the three indexes and their correlation with thalassemia were analyzed. And the diagnostic value of serum HGB, SF and TfR levels for thalassemia was analyzed. Results The levels of HGB and SF in second trimester group and third trimester group were lower than those in healthy control group, while TfR level was higher than that in healthy control group (P<0.05). The levels of HGB and SF in third trimester group were lower than those in second trimester group, while TfR level was higher than that in second trimester group (P<0.05). The level of serum TfR was negatively correlated with levels of serum HGB and SF, while level of serum HGB was positively correlated with SF level (P<0.05). The levels of serum HGB and SF were negatively correlated with the occurrence of thalassemia, while level of serum TfR was positively correlated with it (P<0.05). ROC curve analysis showed that AUC of serum HGB combined with SF and and TfR for diagnosis of thalassemia was greater than that of single index (P<0.05). Conclusion The combined detection of serum HGB, SF and TfR levels is of diagnostic value for mild thalassemia in the second and third trimesters of pregnancy.

摘要: 目的 探讨血清血红蛋白(HGB)、铁蛋白(SF)、转铁蛋白受体(TfR)在妊娠中期与晚期轻型地中海贫血患者中诊断价值。方法 选取2017年8月至2019年7月在我院治疗妊娠中期及晚期轻型地中海贫血患者各40例,并以同期健康孕检者40例为健康对照组。比较三组血清HGB、SF、TfR水平;分析血清HGB、SF、TfR水平间的相关性及其与患者发生地中海贫血的相关性;分析血清HGB、SF、TfR水平对地中海贫血的诊断价值。结果 妊娠中期、妊娠晚期组HGB、SF水平低于健康对照组,TfR水平高于健康对照组(P<0.05);妊娠晚期组HGB、SF水平低于妊娠中期组,TfR水平高于妊娠中期组(P<0.05);血清TfR水平与血清HGB、SF水平成负相关,血清HGB水平与SF水平呈正相关(P<0.05);患者血清HGB、SF水平与发生地中海贫血成负相关,血清TfR水平与发生地中海贫血成正相关(P<0.05);经ROC曲线分析得,血清HGB、SF、TfR水平联合检测诊断地中海贫血的AUC大于单独检测(P<0.05)。结论 血清HGB、SF、TfR水平联合检测对妊娠期中晚期轻型地中海贫血具有诊断价值。
