The Significance of Dsg1 and Dsg3 Antibody's Detection in Pemphigus Patients

Objective In order to investigate the application of anti-Dsg1and anti-Dsg3 antibody in diagnosis and treatment of patients, the level and changes of anti-Dsg1and anti-Dsg3 antibody in 18 patients with pemphigus and the relationship between the antibody index and the severity of the disease were analyzed. Methods 18 patients were diagnosed for pemphigus by direct immunofluorescence and pathology, and the patient's pathogenetic condition was assessed by ABSIS standard.The level of Dsg1 and Dsg3 of patients and nomals were detected by enzyme-linked immunosorbent assay(ELISA). Statistics was performed to analyze the relationship and significance of each index at last. Results Among the 18 patients, 13 were Dsg1 positive and 10 Dsg3 positive . The index of Dsg1 and Dsg3 after treatment were decreased than before, and the difference was statistically significant (P<0.05). Of the 20 nomals, 2 were Dsg1 positive and 1 was Dsg3 positive. The sensitivity of Dsg1 and Dsg3 antibody detection was 72.22% and 55.56% respectively, and the specificity was 90% and 95% respectively. The index of Dsg antibody was correlated with the ABSIS score (ALL P<0.05). Conclusion The Dsg antibody level of patients with pemphigus was correlated with the severity of the disease with high sensitivity and specificity. The detection of Dsg antibody level can be used for diagnosis and guidance of state and treatment of the disease.

Diagnosis and Treatment of Lung Cancer with Hyponatremia as the First Symptom and Literature Review

Objective To explore the diagnosis and treatment process, clinical manifestation and outcome of lung cancer patients with hyponatremia as the first symptom. Methods Retrospective analysis of 4 patients with hyponatremia. Results Four patients with lung cancer were first diagnosed with hyponatremia, their serum sodium was between 112~122mmol/L, and their plasma osmotic pressure was low and urine osmotic pressure was high, so they were diagnosed as SIADH. The treatment of water restriction and sodium supplement can significantly increase the concentration of serum sodium. Through follow-up, we know that 3 patients with primary disease are in stable condition after active treatment, but the other one patient has died. Conclusion The clinical manifestation of SIADH, caused by lung cancer is hyponatremia. This provides clues for the diagnosis of lung cancer. However, the incidence of the disease is hidden, if early detection and active treatment will help to improve the prognosis of patients.

The Correlations Between CIMT in Patients with Type 2 Diabetes Mellitus and Omentin-1, Visfatin and Glutamyl transpeptidase

Objective To investigate the correlations between carotid intima media thickness (CIMT) in patients with type 2 diabetes mellitus and omentin-1, visfatin, glutamyltranspeptidase. Methods The 116 patients with type 2 diabetes mellitus were divided into thickening group (CIMT≥0.9mm) and non-thickening group (CIMT<0.9mm) according to the CIMT. The omentin-1, visfatin and glutamyltranspeptidase in serum of patients in 2 groups were measured. Results The CIMT and visfatin of patients in thickening group were significantly higher than those in non-thickening group (P<0.05), the omentin-1 were significantly lower (P<0.05). There was no significant difference in glutamyltranspeptidase between the two groups (P>0.05). Spearman correlation analysis showed that the CIMT of type 2 diabetic patients negatively correlated with omentin-1 (r =–0.726, P<0.05), but positively correlated with visfatin (r = 0.919, P<0.05). Multivariate logistic regression analysis showed that visfatin was the independent risk factor for CIMT thickening in type 2 diabetic patients. Conclusion Omentin-1 and visfatinare correlated with CIMTin patients with type 2 diabetesmellitus, which can be used as serological indexs for early diagnosis of carotid atherosclerosis.

RELATIONSHIPS BETWEEN BONE TURNOVER MARKERS AND DIABETIC MICROVASCULAR COMPLICATIONS IN TYPE 2 DIABETES MELLITUS

Objective: To investigate the relationships between bone turnover markers and diabetic microvascular complications in type 2 diabetes mellitus. Methods: 146 type 2 diabetes mellitus patients were divided into experimental group (77 patients, complicated with diabetic nephropathy and/or diabetic retinopathy) and control group (69 patients, no diabetic microvascular complications). The general informations of patients in 2 groups were recorded; and the blood biochemical indicators and bone turnover markers of patients were detected. Results: The course of disease of patients in experimental group was remarkably longer than control group (P<0.05); the carboxy-terminal collagen crosslinks (CTX) of patients in experimental group was remarkably higher than control group, the osteocalcin (OC) was remarkably lower (P<0.05). Logistic regression analysis showed that CTX and course of disease were risk factors of type 2 diabetes mellitus with microangiopathy, OC was protective factor. Conclusions: Bone turnover markers CTX and OC are associated with the risk of microvascular complications in patients with type 2 diabetes mellitus, which are expected to be markers for monitoring microangiopathy and bone metabolism in patients with type 2 diabetes mellitus.
