Resting-state Functional Magnetic Resonance Study of Temporal Epilepsy Comorbidity Depression

Objective To study the effect of TLE brain dysfunction on depression by analysing the brain electrical activity, and find out the functional specific brain regions related to TLE complicated with depression, so as to further assist clinical diagnosis and treatment. Methods According to Hamilton depression scale score, a total of 30 patients with temporal lobe epilepsy were collected and divided into 15 patients with temporal lobe epilepsy comorbidities depression group and 15 patients with non-depression group. All the 30 patients underwent VEEG examination, 3.0T MRI and RS-fMRI examination, and then the image data were preprocessed and statistically analyzed. Results There were no significant differences in age, sex between the two groups (P>0.05). The difference in duration of epilepsy was statistically significant(P<0.05). Dominant side discharge was more common in depression group. The independent t-test of two samples which based on three kinds of RS-fMRI analysis methods(amplitude of low frequency fluctuations, fractional alff and regional homogeneity) showed that stronger activate brain regions were included the cingulum, the precuneus, the parietal which belonged to default-mode network. Conclusion Depression patients with temporal lobe epilepsy comorbidities generally have a longer course of disease, and more dominant hemispheres are involved. The activation of the brain default network and the abnormal structure of brain regions are related to the generation of depression. The technique can be applied to guide diagnosis, treatment and surgical intervention.

Effect of Trigger Point Therapy Combined with Core Stability Training on CLBP

Objective To investigate the clinical efficacy of trigger point therapy combined with core stability training in the treatment of chronic nonspecific low back pain (CLBP). Methods Seventy-two patients with CLBP were randomly divided into control group and experimental group with 36 cases in each group. Patients in control group were treated with TDP magic lamp irradiation, The experimental group received trigger point therapy combined with core stability training on the basis of the control group, 5 times a week for 2 weeks. The total effective rate of the 2 groups was evaluated after treatment, and the visual analogue scale (VAS) and Oswestry Disability Index (ODI) scores were evaluated before and after treatment. One month after treatment, VAS score and ODI score were followed up between the two groups, and the differences in the above evaluation contents were compared. Results Before treatment, there was no significant difference in VAS score and ODI score between the two groups (P>0.05). After treatment, there were significant differences in the total effective rate, VAS score and ODI score between the two groups (P<0.05). At 1 month follow-up after treatment, the VAS score and ODI score in the group were significantly different from those before and after treatment (P<0.05), and the VAS score and ODI score were significantly different between groups (P<0.05). Conclusion Trigger point therapy combined with core stability training can reduce pain and improve lumbar function of patients with significant clinical efficacy.

Clinical Treatment and Prevention of Ureteral Stents Retention

Objective Removing remained or forgotten ureteral stents is challenging. We present our experience with heavily encrusted ureteral stents and discuss the endourologic treatment options as well as their effectivity. Methods 17 patients were collected and treated at our clinic. Medical records of the patients were retrospectively reviewed and analyzed. Results Seventeen patients (mean 51.82±17.98 years of age) with forgotten stents (mean 16.06±13.54 months of indwelling). According to the Forgotten-Encrusted-Calcified (FECal) classification, all patients were above grade Ⅲ (100%), including grade Ⅴ(7/17,41.2%),grade Ⅳ(7/17,41.2%),grade Ⅲ(3/17,17.6%). All patients were undergoing the operation and removed the stent successfully. In this study, 5 patients(5/17,29.4%)were analyzed for the composition of the stone. Four patients were phosphate calcium and the other one was oxalate calcium. Analysis of the causes of double J tube retention was mainly due to lack of understanding of the disease and forgetting caused by busy work. Meanwhile, 58.8% of the patients hoped to be supervised and reminded by the hospital to avoid the occurrence of stent retention. Conclusion Endoscopic treatment of ureteral stent retention is safe, minimally invasive and effective. The whole process of health management is one of the helpful way to prevent stent retention after urological surgery.

Preparation and Evaluation of Ursolic Acid Solid Dispersions

Objective To establish preparation methods of ursolic acid solid dispersions and evaluate the quality in vitro. Methods Ursolic acid solid dispersions (UA-SD) were obtained with different carriers by solvent method. Based on crystal suppression experiment, the carrier materials were screened out. The dissolution rate of UA-SD with different drug loading ratio was compared by dissolution test, and the optimal UA-SD drug loading ratio was screened then the solid dispersion was identified by scanning electron microscopy (SEM), differential scanning calorimetry (DSC), X-ray diffraction (X-RD) and fourier transform infrared spectroscopy (FT-IR). Results Soluplus was selected as the carrier of the preparation UA-SD by crystal suppression experiments. The dissolution in vitro showed that the cumulative dissolution rate of UA-SD at 120 minute was 102.7% with a mass ratio of drug to carrier of 1:11. Solid dispersion displayed a much higher dissolution rate than raw medicine and physical mixture. The SEM, DSC, XRD and FT-IR characterizations showed that UA exist in solid dispersions in amorphous form. Conclusion Preparation of UA-SD with Soluplus as carrier can significantly increase the dissolution rate of ursolic acid.

The Necessity of Indwelling Ureteral Stents Before Soft Ureteroscopy Based on Real World Study

Objective To evaluate the therapeutic effectiveness and safety of flexible ureteroscopes without preoperative ureteral stent placement based on the real world study. Methods A total of 351 patients who underwent flexible ureteroscopy (FURS) lithotripsy in The Urology Department of Chengdu Fifth People's Hospital from January 2018 to November 2021 were collected. Based on the real world study, 293 patients without preoperative indwelling of ureteral stents were divided into non-stented group, and the remaining 58 patients with preoperative indwelling of ureteral stents were divided into pre-stented group under the premise of fully informing patients whether to choose preoperative indwelling of ureteral stents.. The operative time, length of hospital stay, stone-free rates (SFRs) (3 days post-operation and 30 days post-operation), complications, and hospital costs of the two groups were respectively compared. Results The average operative time of Pre-stented group was longer than that of Non-stented group (P=0.1680), the SFRs of Pre-stented group were significantly higher than those of Non-stented group at 3 days postoperatively(P=0.0323). However, no significant difference was found in the SFRs between two group 30 days postoperatively(P>0.05), and no difference in complications rates between two groups. The average length of hospital stays and the average hospital costs were substantially lower in Non-stented group than those in Pre-stented group(P<0.05). Conclusions These results indicated that preoperative ureteral stent placement is not necessary in FURS , and without pre-stented ureteral stent is effective and safe for the treatment of upper urinary calculi through FURS.

miR-638 Targets Down-Regulation of MACC1 to Inhibit the Malignant Progression of Gastric Cancer Cells

Objective To explore the biological functions and specific molecular mechanisms of miR-638 in gastric cancer. Methods The mRNA expressions of miR-638 and MACC1 in gastric cancer cells were detected by QRT-PCR, and the protein expression of MACC1 in gastric cancer cells was detected by Western blot. The dual luciferase experiment verified the targeted binding relationship between MACC1 and miR-638. The CCK8 method was used to determine the cell proliferation activity. The scratch healing test and transwell test were used to evaluate its migration and invasion capabilities. Flow cytometry experiments to detect cell cycle distribution. Results miR-638 is down-regulated in gastric cancer cells. Overexpression of miR-638 inhibits the proliferation, migration and invasion of gastric cancer cells. MACC1 expression is significantly up-regulated in gastric cancer cells, miR-638 targets the inhibition of MACC1 expression. miR-638 inhibits the proliferation, migration and invasion of gastric cancer cells by targeted down-regulation of MACC1. Conclusion miR-638 inhibits the occurrence and development of gastric cancer cells by targeting down-regulation of MACC1.

Relevance Study of Neutrophil to Lymphocyte Ratio and Platelet to Lymphocyte Ratio for Carotid Stenosis

Objective To investigate the relationship between the neutrophil to lymphocyte ratio (NLR), the platelet to lymphocyte ratio (PLR) and the degree of carotid stenosis. Methods The 236 patients who received angiography were selected as study objects. According to the angiography results, the patients were divided into the pathological group and the non-pathological group, and the NLR and PLR of the two groups were compared. The relationship between NLR and PLR and the degree of carotid artery stenosis was analyzed according to the degree of carotid artery stenosis of the patients, and ROC curve was plotted. Results Compared with patients without carotid artery stenosis, PLR and NLR increased in patients with carotid artery stenosis (P<0.05), with statistically significant differences. NLR and PLR were positively correlated with the degree of carotid artery stenosis in the patients(P<0.05), indicating a statistically significant difference. With severe and occlusion of carotid stenosis of PLR and NLR ROC curve analysis and calculate the area under the curve (motorcycle under the curve, AUC). PLR and NLR have predictive value (P<0.05), the AUC of PLR is 0.613, the best capture a value of 128.41, sensitivity of 63%, specificity of 58.7%, the AUC of NLR is 0.616, the best capture a value of 2.76, sensitivity of 52.1%, specificity of 74.6%. Conclusion NLR and PLR are valuable in predicting the degree of carotid stenosis.

CLINICAL ANALYSIS OF DIAGNOSING AND TOTAL THORACOSCOPIC SURGERY FOR MEDIASTINAL CYSTS OF 32 CASES

Objective: To explore the clinical features, diagnosis and thoracoscopic surgery key points of mediastinal cysts. Methods: The clinical data, diagnosis and thoracoscopic surgery key points of 32 mediastinal cysts patients that were treated with totally thoracoscopic surgery from 2011.1 to 2018.4 were retrospectively analyzed. Results: Preoperative CT showed that 27 cysts in anterior mediastinum, 1 in middle mediastinum, 4 in posterior mediastinum; and the longest diameter of the cysts was (29.83±17.59)mm. The plain scan CT value of cysts was (25.03±20.03)HU, the enhanced CT value was (33.10±18.56)HU.The CT value of cysts in plain and enhanced scans were not statistic different (t=-1.603, P=0.114). All the 32 patients were treated with thoracoscopic surgery, the average operative time was (104.33±20.51)min, the intraoperative blood loss was (20.15±17.20)ml, and no perioperative deaths and no patients of conversion to open surgery. The pathological diagnoses after operation were 25 cases thymic cyst, 2 cases pericardial cyst, 2 cases esophageal cyst, 1 case cystic mature teratoma, 1 case bronchial cyst and 1 case cystic schwannoma. The mean follow-up time of 32 patients was 3.2 years, and no deaths and recurrent cases. Conclusions: Thoracoscopic surgery is an effective method for diagnosing and treating of mediastinal cyst, and has the advantages of safety, reliability and minimal invasiveness.

ARTHROSCOPIC TREATMENT OF DISCOID MENISCUS INJURY OF KNEE JOINT AND ANALYSIS OF RISK FACTORS FOR POSTOPERATIVE PAIN

Objective: To analyze the clinical effects for treating discoid meniscus injury of knee joint with arthroscopy and the risk factors for postoperative pain. Methods: The clinical data of 30 discoid meniscus injury of knee joint patients were retrospectively analyzed. All the patients were treated with arthroscopic meniscus plasty, the curative effects and risk factors for postoperative pain were analyzed. Results: The patients were followed up for 12 months after operation, and the good rate was 93.3% (28/30) according to Lkeuchi score. The postoperative pain of patients was not related to sex and BMI (P>0.05); but related to age, loading time, articular cartilage injury and cold compress (P<0.05). Conclusions: Treating discoid meniscusinjury of knee joint with arthroscopy can get remarkable short-term effects; clinical attention should be paid to the factors related to postoperative pain, such as age, loading time, articular cartilage injury and cold compress.
