Single-center Study of Serum Anti-PLA2R Antibodies in Adults with Idiopathic Membranous Nephropathy with Clinically Significant Cut-off Value

Objective To explore the clinically significant cut-off value of serum anti-PLA2R antibody (sPLA2R-Ab) in adult idiopathic membranous nephropathy (IMN) in Baoding City. Methods A total of 285 patients who underwent renal needle biopsy and sPLA2R-Ab detection in Baoding First Central Hospital from January 2018 to December 2021 were included. The patients were divided into IMN group, secondary membranous nephropathy (SMN) group and other groups. The level of sPLA2R-Ab was compared between the groups; the correlation between sPLA2R-Ab level and other clinical indicators in IMN patients was analyzed; logistic regression was applied to identify the risk factors of IMN; the diagnostic value of sPLA2R-Ab for IMN was analyzed by ROC curve, and Youden index was calculated to obtain the optimal cut-off value of sPLA2R-Ab for IMN diagnosis. Results The level of sPLA2R-Ab in IMN group was significantly higher than that in SMN group and other groups(P<0.05). In IMN group, sPLA2R-Ab was negatively correlated with total protein and albumin(P<0.001), it was positively correlated with 24h urinary protein, serum creatinine and cholesterol(P<0.05). ROC curve showed that the cut-off value of sPLA2R-Ab in the diagnosis of IMN patients was 3.315RU/mL, the corresponding Youden index was the highest (0.651), the sensitivity was 79.4%, the specificity was 85.7%, the AUC was 0.829, P<0.001. Conclusion sPLA2R-Ab is associated with the activity of IMN disease. The cut-off value of SPLA2R-AB for differentiating IMN from non-IMN patients is 3.315RU/mL, which can improve the early diagnosis rate of IMN.

Detection of Chronic Stress Anxiety Behavior Induced by Isoproterenol

Objective To elucidate the long-term effect of isoproterenol on inducing anxiety-like behavior under chronic stress in kunming mice. Methods A total of 50 Kunming mice were randomly divided into control, chronic stress, isoproterenol (ISO) group, chronic stress +ICI 118, 551 (ICI) group and ISO+ICI 118, 551(ISO+ICI) group (n = 10 in each group). Control group: normal saline was intraperitoneally injected every day; Chronic stress group: Chronic stress treatment was given every day. ISO treatment group: ISO (5 mg/kg) was injected intraperitoneally every day. Chronic stress +ICI treatment group: ICI (0.2 mg/kg) was injected intraperitoneally every day after chronic stress treatment. ISO+ICI treatment group: ISO(5mg/kg) and ICI(0.2 mg/kg) were given intraperitoneally every day. Results The open arm residence time, the number of entering open arm, the percentage of open arm residence time and the number of entering open arm in the chronic stress group and ISO group were significantly lower than those in the control group (P<0.05). The open-box retention time, percentage of open-box retention time, open-box activity times and shuttle times in the chronic stress group and ISO treatment group were significantly lower than those in the control group (P<0.05). The retention time of dark box was significantly higher than that of the control group (P<0.05). Conclusion After long-term treatment with β-AR agonist isoproterenol, kunming mice developed anxiety-like behavior under chronic stress.

EFFECTS OF LABETALOL HYDROCHLORIDE COMBINED WITH FOLIC ACID IN THE TREATMENT OF EARLY-ONSET PREECLAMPSIA OF 48 CASES

Objective: To explore the treatment effects of labetalol hydrochloride combined with folic acid on early-onset preeclampsia and the effects on maternal-infant outcomes. Methods: Ninety-six patients with early-onset preeclampsia were randomly divided into study group and control group with 48 cases in each group. The patients in control group were treated with conventional treatment and folic acid; On the basis of the control group, the patients in study group were also give labetalol hydrochloride. The blood pressure [systolic blood pressure (SBP), diastolic blood pressure (DBP)], uterine hemodynamics indexes [pulsatility index (PI), resistance index (RI)] before and 1week after treatment, as well as the maternal-infant outcomes (natural delivery, postpartum hemorrhage, placental abruption, neonatal asphyxia, neonatal Apgar score) of patients in 2 groups were compared. Results: 1 week after treatment, the SBP, DBP, PI and RI of patients in 2 groups were significantly decreased compared with those before treatment (P<0.05); And the above indexes of patients in study group after treatment were significantly lower than those in control group after treatment (P<0.05). There were no significant statistical significances about the incidence rates of postpartum hemorrhage, placental abruption and neonatal asphyxia between the two groups (P>0.05). The natural delivery rate and neonatal Apgar score of patients in study group were significantly higher than those in control group (P<0.05). Conclusions: Labetalol hydrochloride combined with folic acid has better effects on treating early-onset preeclampsia, and it can control blood pressure, improve uterine blood supply and neonatal physical condition at birth effectively.
