Objective
To establish a predictive model for predicting the prognosis of patients with idiopathic membranous nephropathy (IMN) by Nomogram diagram.
Methods
The newly diagnosed patients with IMN diagnosed by renal biopsy in The Affiliated Hospital of Chengde Medical University from January 2018 to December 2020 were selected and followed up for 24 months. The patients were divided into two groups according to whether the patients had renal endpoint events at the end of follow-up. Patients who did not have terminal events at the end of follow-up were enrolled in the group that did not reach the renal endpoint. The influencing factors with P<0.2 in univariate Logistic analysis were analyzed by multivariate Logistic regression analysis, and the optimal Logistic regression model was selected according to Akaike information criterion (AIC) to construct the predictive model of poor prognosis of IMN patients. The discrimination ability of the model was verified and evaluated by the area under the receiver operating characteristic (AUROC), and the calibration curve and decision curve analysis (DCA) were drawn to evaluate the calibration, clinical net income and practicability of the model.
Results
The predictive factors used to predict the poor prognosis of IMN include age, mean arterial pressure(MAP), duration of disease before renal puncture, albumin and serum creatinine. The AUROC of patients with IMN was 0.729. The statistical value of the Hosmer-Lemeshow test of the calibration curve is 1.44 (P=0.49). DCA curve shows that this model is clinically applicable when the predictive probability of IMN is between 0.17 and 0.44.
Conclusion
In this study, a predictive model for predicting the poor prognosis of IMN patients was constructed. The predictive model has good predictive ability, calibration ability, and clinical net benefit, it can be applied to predict the prognosis of IMN patients.

Objective
To investigate the efficacy and safety of Cinobufacini combined with chemotherapy in the treatment of colorectal cancer (CRC).
Methods
By searching the publicly published case-control studies on the combination of Cinobufacini in the treatment of colorectal cancer, RevMan 5.3 analysis software was used for systematic evaluation.
Results
A total of 10 research data were included. Meta-analysis showed that compared with conventional chemotherapy, the combination of cinobufagin and chemotherapy drugs could significantly improve the chemotherapy effect of patients (P=0.0001), the quality of patients life, KPS score (P<0.00001), and relieve pain (P<0.0003). In addition, a series of adverse reactions caused by chemotherapy were significantly reduced in colorectal patients (P<0.05).
Conclusion
Cinobufacini combined with chemotherapy has a more significant clinical effect than chemotherapy alone in the treatment of colorectal patients.

Objective
To systematically evaluate the correlation between mTOR, p-mTOR and colorectal cancer and its clinicopathological characteristics.
Methods
Through searching domestic and foreign published articles on the relationship between mTOR, p-mTOR and colorectal cancer, the NOS scale was used to evaluate the risk of bias in the study, and the sensitivity and heterogeneity of the included literature were analyzed by Rev Man 5.3 software.
Results
Compared with normal or adjacent tissues, the expression of mTOR and p-mTOR was higher in colorectal cancer tissues (P<0.00001); compared with the infiltration depth T1 and T2 groups, mTOR and p-mTOR were in the infiltration depth T3, The expression of the T4 group was significantly increased (P<0.00001); compared with the clinical stage Ⅰ~Ⅱ group, the expression of mTOR and p-mTOR was higher in the clinical stage Ⅲ~Ⅳ group (P<0.00001); and no lymph node metastasis Compared with the group, the expression of mTOR and p-mTOR in the group with lymph node metastasis was significantly increased (P<0.0001); compared with the group without distant metastasis, the expression of mTOR and p-mTOR in the group with distant metastasis was significantly increased (P<0.0001); However, the expression of mTOR and p-mTOR was not statistically different between the poorly differentiated group and the middle-highly differentiated group.
Conclusion
The expression of mTOR and p-mTOR play an important role in the occurrence and development of colorectal cancer.

Objective:
To study the expression and clinical significance of miR-15a-5p and multidrug resistance gene 1(MDR1) mRNA in colorectal carcinoma tissues.
Methods:
RT-qPCR was used to detect the expression of miR-15a-5p and MDR1 mRNA in 44 cases of colorectal carcinoma and adjacent non-tumor tissues. The relationships between miR-15a-5p, MDR1 mRNA and clinical pathological features of colorectal carcinoma, as well as the correlations between miR-15a-5p and MDR1 mRNA in colorectal carcinoma were also analyzed.
Results:
Compared with the adjacent non-tumor tissues, miR-15a-5p mRNA expression was significantly lower in colorectal carcinoma, but MDR1 mRNA expression was significantly higher (P<0.05). The miR-15a-5p and MDR1 mRNA expression were all concerned with lymph node metastasis and TNM stage of colorectal carcinoma (P<0.05). In colorectal carcinoma, miR-15a-5p mRNA expression was negatively correlated with MDR1 mRNA expression (r=-0.343, P<0.05).
Conclusions:
The expression of miR-15a-5p was low and MDR1 was high in colorectal cancer tissues. Both of them may play an important role in development of colorectal carcinoma.
