Correlation between METmRNA Expression in Carotid Artery Plaques and Ischemic Cerebrovascular Disease

Objective To investigate the correlation between the expression of MET mRNA in carotid artery plaques and the occurrence of ischemic cerebrovascular disease (ICVD), in order to provide reference for the prevention and treatment of ischemic cerebrovascular disease. Method 97 ICVD patients who underwent carotid endarterectomy in our hospital from January 2021 to March 2023 were included. They were divided into vulnerable, non vulnerable, and coexisting groups based on the nature of the detected plaques. The MET mRNA of the stripped plaques was detected using qRT PCR method, and the correlation between MET mRNA and plaque nature, ICVD subtypes, and ICVD severity was analyzed. Results The mRNA expression levels in the vulnerable group, non vulnerable group, and coexisting group were 0.763 ± 0.047, 0.591 ± 0.039, and 0.708 ± 0.043, respectively, with statistically significant differences (P<0.05). The mRNA expression levels among patients with transient cerebral ischemia, cerebral infarction, cerebral steal syndrome, and chronic cerebral ischemia were 0.711 ± 0.068, 0.765 ± 0.077, 0.655 ± 0.071, and 0.671 ± 0.083, respectively, with statistically significant differences (P<0.05). The mRNA expression levels among patients with mild, moderate, moderate, and severe NIHSS grading were 0.642 ± 0.072, 0.671 ± 0.074, 0.754 ± 0.039, and 0.789 ± 0.051 respectively, with statistically significant differences (P<0.05). The expression level of MET mRNA in carotid plaques was significantly correlated with plaque type, ICVD classification, NIHSS grading, carotid stenosis grading, fasting blood glucose, total cholesterol, triglycerides, high-density lipoprotein, and low-density lipoprotein, all P<0.05. Conclusion The expression of MET mRNA in carotid artery plaques is significantly correlated with the occurrence of ICVD, mainly by participating in the development of plaque instability leading to transient cerebral ischemia and stroke, and interacting with hyperglycemia and hyperlipidemia.

Comparison Between Lymph Node Dissection Through Low-collar Incision and L-shaped Incision for Thyroid Cancer

Objective To investigate the application of lymph node dissection through a low-collar incision and a L-shaped incision to treat patients with thyroid cancer. Methods Ninety-two patients with thyroid cancer who were admitted to Xinyang Central Hospital between April 2017 and May 2020 were selected. Patients were randomly divided into control group and observation group with 46 patients in each group. Patients in the control group were treated with lymph node dissection through a L-shaped incision, and those in the observation group were treated with lymph node dissection through a low-collar incision. Surgical indicators, satisfaction with incision appearance, neck and shoulder pain, psychological status, complications, and prognosis were compared between the two groups. Results The operation time, length of incision, intraoperative blood loss, and hospital stay of the observation group were shorter or less than those of the control group (P<0.05). The satisfaction rate with incision appearance in the observation group was 93.48% , higher than 78.26% in the control group (P<0.05). The observation group had lower neck and shoulder pain score, anxiety score, and depression score than that of the control group (P<0.05). The lymph node metastasis rate and lymph node recurrence rate in the observation group were 0.00% and 2.17%. Compared with those in the control group (4.35% and 2.17%), the differences were not statistically significant (P>0.05). Conclusion Lymph node dissection through a low-collar incision can achieve better clinical effects in patients with thyroid cancer, especially in terms of complications and aesthetic effects.

Clinical Value of CT-guided Percutaneous Pulmonary Biopsy

Objective To discuss the clinical value of CT-guided percutaneous pulmonary biopsy. Methods We retrospectively analysed 429 patients who underwent CT-guided percutaneous pulmonary biopsy of Affiliated Hospital of Chengde Medical University from January 2015 to December 2019, and analyzed the puncture accuracy, incidence of major complications and influencing factors. Results All 429 patients completed lung biopsy successfully, 344 patients were diagnosed as malignant lesions, 84 patients were diagnosed as benign lesions, 2 patients had no definite diagnosis, the accuracy rate is 98.3%. The main complications were pneumothorax and pulmonary hemorrhage, with pneumothorax in 86 cases(20.0%), hemoptysis in 77 cases(17.9%) and pericardial effusion in 1 case(0.23%). Pneumothorax complications were related to the depth of injection, and the incidence of pneumothorax was higher when the needle depth was more than 5cm. Conclusion CT-guided percutaneous pulmonary biopsy is helpful for the diagnosis of lung space-occupying lesions with high accuracy, and it is one of the important methods for the definite diagnosis of lung lesions.

Short-Term Exhaustive Exercise Regulates Cell Electrophysiological Properties in Rat Ventricular Myocytes

Objective To investigate the effect of exhaustive exercise on sodium channel currents (INa), ECG and echocardiography parameters in a rat swimming model. Methods Rats of the exercise group were forced to swim until exhaustion each time for 9 days with 5% body weight (workload) attached to the head. A swimming session will be held at 9 am from day 1 to day 9. The criteria of exhausting exercise included the rotatingmotion in water, the significant decrease in motor coordination, and more than 10 seconds from water submerged the nose to the body resurfaced. After 1 day of recovery period, we performed ventricular myocytes isolation and used whole cell patch-clamp technique to investigate sodium channel currents, sedentary rats served as controls. In addition, echocardiography and ECG parameters were analyzed. Results After 9 days of swimming exercise, a decrease in EF and FS% was observed, however, no significant differences were found between sedentary and exercise group. In addition, no significant differences were found in heart rate, LVPWs, LVPWd, IVSs, IVSd, LVIDs, LVIDd, EDV, LV Mass between sedentary and exercise group. The following ECGs parameters were increased: P wave amplitude, P wave duration, QRS amplitude and R wave amplitude, while the other parameters remained unchanged. When compared to sedentary, peak INa density was enhanced by exhaustive exercise, however, exhaustive exercise didn't change the steady-state activation and inactivation of INa. Conclusion The P, QRS and R-wave amplitudes increased after exhaustive exercise indicating impaired myocardial depolarization, which may be caused by the enhanced INa. These changes could shorten the action potential, and thus may contribute to proarrhythmia in cardiomyocytes.
