EGFR在口腔鳞状细胞癌中的表达及临床意义

Expression and clinical significance of EGFR in oral squamous cell carcinoma

摘要: 目的 探讨口腔鳞状细胞癌(OSCC)组织及细胞中表皮生长因子受体(EGFR)的表达水平及其临床意义。方法 运用免疫组化的方法检测EGFR在108例患者OSCC组织及64例正常组织中的表达,分析其与OSCC临床病理因素及预后之间的关系。利用Western blot和q-RTPCR检测口腔上皮细胞系和OSCC细胞系中EGFR蛋白和mRNA水平。结果 EGFR在OSCC组织中高表达,显著高于正常组(P<0.05)。EGFR用于诊断OSCC的ROC曲线下面积(AUC)为0.838。EGFR的表达与OSCC分化程度和浸润深度有关(P=0.027,P=0.041)。根据Kaplan-Meier和COX回归分析结果发现,患者的预后受到EGFR表达水平、肿瘤分化程度、T分期以及TNM分期的影响(P<0.01)。WB和qRT-PCR结果显示,EGFR的蛋白及mRNA表达水平在OSCC细胞系中显著高于正常口腔上皮细胞(P<0.05)。结论 EGFR在OSCC中高表达,是OSCC潜在的诊断及预后不良标记物。

Abstract: Objective To investigate the expression profile of epidermal growth factor receptor (EGFR) in OSCC tissues and cell lines, and to elucidate its clinical significance and prognostic value. Methods EGFR expression was characterized in 108 OSCC and 64 adjacent normal tissues via immunohistochemistry (IHC). Correlations between EGFR levels and clinicopathological parameters, as well as patient survival, were statistically analyzed. Furthermore, Western blot and quantitative reverse transcription-polymerase chain reaction (qRT-PCR) were employed to quantify EGFR protein and mRNA levels across a panel of oral epithelial and OSCC cell lines. Results EGFR expression was significantly upregulated in OSCC tissues compared to normal controls (P<0.05). Receiver operating characteristic (ROC) analysis yielded an area under the curve (AUC) of 0.838, indicating robust diagnostic efficacy. High EGFR expression significantly correlated with tumor differentiation grade (P=0.027) and depth of invasion (P = 0.041). Kaplan-Meier analysis and Cox proportional hazards regression identified EGFR expression, differentiation grade, T-stage, and TNM stage as independent prognostic indicators (P<0.01). Consistently, both protein and mRNA levels of EGFR were significantly elevated in OSCC cell lines relative to normal oral epithelial cells (P<0.05). Conclusion EGFR is highly expressed in OSCC and may serve as a potential diagnostic marker and an indicator of poor prognosis.
